Description

  • Gastroesophageal reflux disease (GERD) is one of the most common problems faced by clinicians in their day-to-day practice. Besides life style modifications and general measures, inhibition of gastric acid secretion combined with prokinetic gastrointestinal agents can help prevent reflux and have an important role to play in the management of GERD.
  • Pantoprazole is a substituted benzimidazole, which inhibits the secretion of hydrochloric acid in the stomach by specific blockade of the proton pumps of the parietal cells. Since Pantoprazole binds to the enzyme distal to the cell receptor level, it can inhibit hydrochloric acid secretion independently of the stimulation by other substances (acetylcholine, histamine, and gastrin).
  • Domperidone is a dopamine antagonist with anti-emetic properties. Its antiemetic effect may be due to a combination of peripheral (gastrokinetic) effects and antagonism of dopamine receptors in the chemoreceptor trigger zone, which lies outside the blood-brain barrier in the area postrema. Studies in humans have shown oral domperidone to increase lower oesophageal pressure, improve antroduodenal motility, and accelerate gastric emptying. There is no effect on gastric secretion.
